SUPPORT COORDINATION
Ellisville State School provides Support Coordination services to families to promote independence and community integration and enable persons with disabilities to remain in their community. This is accomplished by:
- Monitoring Services
- Locating Providers
- Providing Access
- Educating Families
- Advocacy
WHO IS QUALIFIED?
Persons with intellectual and developmental disabilities who are medically eligible and require the level of care provided in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) are eligible for support coordination services. There is no cost to the persons receiving services or their families. Ellisville State School determines the eligibility for services by an evaluation. Services are provided in the following counties:
